Terms and conditions for $110 for 5 classes per month-Auto Pay, 3 month minimum contract required

MONTHLY 4 class (5th class free if purchased before 9/30) AUTO-RENEW ENROLLMENT AND AGREEMENT FORM Our Monthly Auto-Pay is our best package with 5 classes per month with the convenience of automatic monthly renewal. Our reduced price for this membership is charged to your credit/debit card on file once a month. This offer comes with a 3 month minimum commitment where no cancellations are permitted during the first 3 months. After the initial 3 months, unless a cancellation form is submitted, your credit/debit card will be automatically charged on a monthly basis. After the initial 3 months, to freeze or cancel, you will need to inform us in writing at least 14 days in advance of your next billing date in order for your request to be effective for the following month. Your billing date is based on your Auto-Pay Activation Date listed below. Other Terms and Conditions: Automatic Monthly Auto-Pay: Renewal terms may be cancelled after the first 3 months provided the client gives the studio 14 days notice in writing. Please submit in person or email to aerialrevolutionstaff@gmail.com. Freeze Options: Freezes are permitted once during the first 3 months: 2 week minimum and one month maximum. After the 3 months, only one freeze is allowed per year. The minimum duration allowed is 1 month with a maximum duration of up to 3 months. There is a $10 per billing date Freeze Fee. This will freeze your payments while you may be out of town. Once the freeze period is over, you monthly payments will resume. Please note that the freeze may shift your Auto-Pay billing date once reactivated. Notify the studio 14 days before you would like the freeze to begin. Notices made in less that 14 days will not be honored. Changing Cards: If for any reason your card has been lost or stolen, please notify the studio immediately. We can easily change the card you are currently using to one that is active in order to avoid any interruption in your membership status. Injuries: If you become injured during your commitment time, you can either freeze your program while you are recovering (this freeze will not be counted against your one time freeze above). We have the right to refuse or discontinue service at any time for any reason: We reserve the right to terminate or deny membership for an indeterminate amount of time if a client has an unsatisfactory payment history. Inappropriate behavior from clients will not be tolerated in any manner. You will be responsible for payment in full upon revocation of membership.

**No shows or late cancels (after 3 hours prior) will result in a loss of class plus a $5 fee. Strictly enforced during covid. Please be respectful to our limitations as a business during covid. Our class sizes are extremely limited. Thanks!
